Required Registration Forms:
Six items are needed to register each player:
- Online Registration for Travel Soccer & payment of $300 - payment will secure your player's spot on their team, and is non-refundable
- NOTARIZED Medical Release Form – (there are notaries at Millburn Town Hall)
- Sage form signed by parent(s) and player
- NJ Youth Soccer Membership Form signed by parent and player
- Copy of the player’s birth certificate or passport – a photocopy is fine
- RECENT, passport sized picture of the player. The player’s head should be no larger than 1 inch. A snapshot is ok.

Required Manager/Volunteer Coach Forms:
Volunteer Managers and other team parents holding at least an "F" license who will be added to a team's roster, must provide the following forms.

- KidSafe Disclosure Form Word File
