Required Player Registration Forms:

Six items are needed to register each player:

1. Online Registration for Travel Soccer & payment of $325 - payment will secure your player's spot on their team, and is non-refundable
2. NOTARIZED Medical Release Form – (there are notaries at Millburn Town Hall)
3. Sage form signed by parent(s) and player
4. NJ Youth Soccer Membership Form signed by parent and player
5. Copy of the player’s birth certificate or passport – a photocopy is fine
6. RECENT, passport sized picture of the player. The player’s head should be no larger than 1 inch. A snapshot is ok.
7. Uniform order Form (new uniforms for 2010/2011 season. Boys and Girls sizes)

• Uniforms must be ordered at Kicks'n Sticks before 7/15 to be assured of having a uniform at the start of the season.

Required Manager/Volunteer Coach Forms:

Volunteer Managers and other team parents holding at least an "F" license who will be added to a team's roster, must provide the following forms.

• Coaches Membership Form  PDF File
• KidSafe Disclosure Form  PDF File
• F license course schedule
